The Indiana Courts website contains a number of legal forms for use in trial and appellate courts.  This website can be found at: www.in.gov/judiciary/forms.  There is also a self-service legal center provided by the Supreme Court for those people who are involved in the court system but do not have a lawyer.  This website can be found at: www.in.gov/judiciary/selfservices.
