Starting February 1, 2012, children with private insurance that cover vaccines (including those that require a deductible, a co-pay, or percentage based coverage) will not be eligible to receive publicly funded vaccine. Parents will have to pay for the vaccine out-of-pocket and have their insurance company reimburse them for the cost. Click here for the health department’s fee schedule.
For those unaware of their insurance’s vaccine coverage should ask their health insurance company the following questions:
What vaccines, if any, does my child’s policy cover? (i.e. all vaccines, only specific vaccines, or no vaccines)
• Is there a yearly dollar limit for vaccine coverage? (i.e. your insurance company only pays for $300 worth of vaccine)
• Is there a co-pay or a deductible?
• Is the policy percentage based? (i.e. do I pay 20% of the cost and my insurance pay the rest)
If the child’s insurance does not include any vaccine coverage, has a yearly dollar cap in vaccine coverage and it has been met, or the insurance only covers specific vaccines, the child is still considered “underinsured” and will be able to use the publicly funded vaccine program.
Children with Medicaid or no insurance are still covered under this program as well.
Childhood immunizations are provided at both of our locations. Immunization administration fees are $10/injection for eligible children, adult rates vary (click here for home page). Medicaid for children is accepted. Please bring your most recent and up to date copy of the immunization record.
Immunizations recommended for international travelers are available at the Health Department in Mishawaka. The clinic is a designated Yellow Fever vaccination center. Appointments are required. Please call (574) 256-6230 for information.
Tuberculosis (TB) Clinic
TB skin tests are offered on Monday, Tuesday, Wednesday, and Friday on a walk-in basis. You must be able to return 48-72 hours after placement of the test. Failure to do so will end in an invalid test. Repeat placement of the test is optional with additional payment required. Our clinic hours of operation are:
