obtain the application for certified birth certificates, which must be filled

- Full name of person whose record is requested
- Date of birth
- Place of birth
- Parents' names, including maiden name of mother
- Only a photocopy of 1 ID is required if it is a Driver's License or State ID. If you do not have a picture ID, please contact our office for acceptable documents.
- Purpose for which copy is needed
- Relationship to person whose record is requested
- Signed letter, return address and phone number
- Send to address below - attention vital records
- Number of copies requested
Prices $10.00 for First Certified Birth Copy $7.00 for each additional copy on the same person at the same time $12.00 for wallet size, including a plastic pouch. No personal checks *Payment must be a money order made payable to the St. Joseph County Health Department. Visa/MasterCard is also accepted, but the request must be on our application.
